When more than one animal is having surgery . . .
INTAKE FORM(S): You can include all the animals having the same type of surgery on the same date and with the same veterinary practice on the same intake form. Example: several kittens being spayed or neutered.
If 2 or more animals are having surgery, but the surgery for each one is different or is being done at difference veterinary facilities, please complete separate intake forms for each animal.
Each time you SUBMIT an Intake Form, you’ll be returned here to make your payment, but you can complete all the forms first, and then make 1 payment for all the Preparation for Surgery sessions.
PAYMENT: Pay for 1 session for 1 animal; 2 sessions for 2 animals; 3 sessions for 3 animals; and so forth.
